APPLICANT'S STATEMENT

The information contained in this application is correct to the best of my knowledge. I authorize any references or churches listed in this application to give you any information (including opinions) that they have regarding my character and fitness for children's ministry. I authorize the release of the information contained in this application to any ministry at NewSpring Church in which I seek a volunteer position. In consideration of the receipt and evaluation of this application by NewSpring Church, I hereby release any individual, church, youth organization, charity, employer, collectively and individually, from any and all liability for damages of whatever kind or nature which may at any time result to me, my heirs, or family, on account of compliance or any attempts or comply with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application. Should my application be accepted, I agree to refrain from unscriptural conduct in the performance of my services on behalf of the church.

I HAVE READ THE FOREGOING RELEASE AND KNOW THAT CONTENTS THEREOF AND SIGN THIS RELEASE AS MY OWN FREE ACT.

NEWSPRING CHURCH
ADULT PRIOR CONSENT TO USE
OF IMAGE, VOICE, & LIKENESS
In consideration of my approved participation in activities organized or sponsored by NewSpring Church, I hereby agree and grant permission to NewSpring Church and any of its licensees and assigns (the "Church") the right to (1) capture and record photographs, video, and/or audio impressions of me and my name, biography, likeness, image, voice, and performance, obtained by or on behalf of the Church from the date listed below until the Church receives written notice of termination from me, and (2) create derivatives of and use the foregoing in any media, whether now known or hereafter developed, including without limitation, internet website, social media sites, CD or DVD labels, or video or film clips, for any and all broadcasting, audio/visual, and/or exhibition purposes, including without limitation, domestic and international television, social media broadcasts, advertisements, or Church publications, at the sole discretion of the Church, in perpetuity, throughout the world.

I understand that once my image appears on the Church's websites, owned or operated television and/or social media websites, or in television or social media broadcasts, the image can be downloaded by any computer user. I expressly release the Church, its officers, directors, employees, volunteers, and agents from any and all claims arising out of the use of my photograph or image. I agree to release the Church, its officers, directors, employees, volunteers and agents from any and all claims arising out of this Agreement or my consent stated in this Agreement.

In consideration of the foregoing I will not be entitled to any royalties, fees, or other compensation or notice, in connection with any such use.

This Agreement represents the entire understanding of the parties and may not be amended unless mutually agreed to by both parties in writing.
